Your First Visit

Patient Forms

For your convenience, our new patient forms are available in our New Patient Center to download and print. You can complete the necessary forms and bring them with you to your appointment. This paperwork provides us with your health history and information on your condition.

Upon entering our office, our staff will collect your paperwork and make a copy of your driver's license and insurance card (if applicable). After you are checked in and your paperwork has been completed, you will begin your consultation with the doctor.


Dr. Platz will sit down with you to review your health history and new patient paperwork. She will then consult with you on your current problem. Any questions or concerns you have along with treatment options will be discussed. During the consultation Dr. Platz will learn more about you, your condition, and your expectations to determine how chiropractic care and help you meet your goals.


After your consultation, Dr. Trista Platz will perform a complete chiropractic evaluation to determine if you are a good candidate for chiropractic care. Examination will include standard physical, neurological, and orthopedic tests, along with postural and range of motion evaluations. However, nothing will be done in our office without your consent.

Diagnostic Imaging (X-rays & MRI)

Depending on your specific condition, we may choose to send you for some diagnostic imaging as well. Diagnostic imaging helps us develop the most effective treatment plan for you and alert us of any serious spinal conditions. Once we receive your imaging, Dr. Platz will go over the results with you.

Report of Findings

Once the information is collected and examinations are performed, Dr. Trista Platz will give you a detailed report of findings and answer any questions including:

  1. What is causing my pain?
  2. How can you help me?
  3. How often do I need to come in?
  4. What will my treatment  cost?

After reviewing your health history, goals, and examining your spine and imaging, Dr. Trista Platz will discuss your treatment options and her recommendations for you. She will explain how chiropractic care could benefit you and notify you if your condition requires care with other health care providers.  Dr. Trista Platz  will provide the best treatment plan for your needs.


At the conclusion of this initial appointment, Dr. Platz may choose to start treatment right away. This may include spinal adjustments, physical therapy, corrective exercises, soft tissue work and/or massage. Treatment is interactive, so you can express concerns about the different styles of treatment anytime. We will do our best to explain the importance of the different aspects of your treatment plan.

Home Care

Prior to leaving, the doctor will suggest a home care plan designed to enhance your treatment and help you improve faster. This may include: ice or heat application instructions, certain activities or positions to avoid, at home exercises, and/or stretches. The main purpose of our home care is to help you achieve good spinal alignment, have less pain, and be able to get back to doing the things you love to do.

Everyone is different and everyone needs a treatment plan that fits in with their lifestyle and their needs. We strive to help you get the most out of your chiropractic care. Our team will work with you to create healthy habits for your lifestyle by educating you on a healthy diet, exercise, and managing stress.
